Admissions

The admission process is the first step in forming a relationship between your family and the West Center. The information needed to begin the process is:

• Completed application.
• Most recent Medical records including diagnosis and test.
• A copy of the Power of Attorney or Guardianship.

The next step will be an assessment by the West Center Admission person to determine the best placement as well as the family choosing the physician they wish to follow the care of their loved one. Occasionally, there is a waiting list depending on the best suite placement and your apartment choice.

There is a deposit amount that is equal to two monthly payments. Half of this deposit is applied against the first month’s stay. The other half is held as a deposit until the resident is discharged from our facility.
